Chad Ruhwedel, Nick Baptiste and Colby Robak scored in regulation for the Americans (9-9-1), with Robak's goal coming at 19:42 of the third period to force overtime.
Morgan Ellis, Daniel Carr and Michael McCarron struck for the IceCaps (9-5-5), who are 3-0-3 in their past six outings. Mark Barberio assisted on all three goals.
Rochester's Andrey Makarov made 29 saves for the win as Zachary Fucale kicked out 31-of-34 shots for St. John's.
The Americans went 1 for 6 on the power play while the IceCaps were 1 for 5.
