AstraZeneca and Ionis reported that the Phase III CARDIO-TTRansform trial of Wainua (eplontersen) in adults with transthyretin-mediated amyloid cardiomyopathy failed to meet its primary endpoint, showing no statistically significant reduction in cardiovascular mortality and recurrent cardiovascular events when added to standard-of-care stabiliser therapy. Wainua was generally well tolerated with a safety profile consistent with prior data, and the companies plan to present full results at the European Society of Cardiology Congress.
In a prespecified subgroup of patients receiving Wainua as monotherapy, fewer primary composite cardiovascular events were observed versus placebo, a result described as nominally significant, while no treatment effect was seen in patients already on stabiliser therapy at baseline. The outcome represents a setback for AstraZeneca’s effort to extend Wainua beyond its existing approvals for hereditary transthyretin-mediated amyloid polyneuropathy, highlighting the challenges of improving outcomes in an evolving ATTR-CM treatment landscape where standard therapies are increasingly entrenched.
